Every fix is tested by the team but only with a limited set of use cases. When one or two persons test a mechanics during 30 minutes, they can't cover every single situation in which that mechanics will be used. The previous fix for charge is a good example, because it was tested in PvE, PvP, with moving and immobile targets, with small and large targets, on flat and bumpy ground... but was not tested underwater.
The last fix was also tested underwater, and in a free fall (when charging ground target from air).
Still, there must be situations in which charge was not tested by devs, but that will happen, especially to no-lifer warriors and with almost 1k warriors logged in at a time (plus druids)

Do you mean that you turn by 90-180 degree angle compared to your initial angle?
Does it happen when you clic (like trying to move) during a charge? It is in PvP, PvE, on moving target only?
