Autohits after gouge

This has happened at least twice now. Maybe more but never in situation where it mattered. I'm interested in why this happens and when.

It's easy to blame lag but it's weird that it's my own inputs that seem desynced.

I'm playing <100ms btw.

Sword Spec / Hand of Justice / Windfury / Thrash Blade / Ironfoe proccing off Gouge may have caused this. I never encountered that issue except with one of these procs - but I don't have any latency issue though.
