Tome of the Cabal missing?

Logged in today to discover the quest Tome of the Cabal plus the items for the quest have vanished from my bags/logs - Has this happened to anybody else?

I've checked my bank there is no quest items related to tome of the cabal in there..
Also had issues with Ledger for Tanaris quest today also, cannot hand in quest with items in bag or in bank..
