On the same/similar topic, I am AV exalted now, but the NPC wont offer the quest to upgrade my Frostwolf trinket to rank 6. Or rank 5. Or rank 4. (It's currently rank 3)

I am experiencing the same problem. I had the Q completed in my Q log and I was about to turn it in but it suddenly dissapeared from my Q log so I have no reward, no quest and now I just can take the follow up quest from NPC.

Appears to be fixed, the quest appeared back in my quest log. If you had the quest already when this happened then check your log, if you didn't have it but couldn't pick it up then go to the quest giver and I imagine it is there.
