They don't. Recently a guy was caught multiboxing, and he got his friend on skype to play his character so it looked like two people playing. This failed, and the guy was perma-banned for account sharing as well.
Bottom line:
If you are AFK, stay AFK. Don't go on /follow. No, it is not illegal to do so, but there indeed IS a risk!
If you get caught, the "My friend is at the toilet" won't be enough.
