Quest item dissapearing

Hi! I just did onyxia with the quest The Forging of Quel'serrar (raid), and my quest item dissapeared. I was hoping you could help me with this issue. I did all the requirements for the quest, like putting down the sword in her breath and killing her. This is really sad, because i've been farmin Foror's compendium of dragonslaying for some time now ! please help me
