Bug report: Quest Draenethyst Crystals not working

Level 35, Alliance Human. Quest: Draenethyst Crystals

Bug: When attempting use the Draenethyst Crystal entity, it does not give a Draenethyst Crystal, and the item moves to a "this is in use" state.

How do I file a proper bug report?

Confirmed. Having the exact same issue today. I've tried around 10 different nodes and none of them yield a crystal for the Draenethyst Crystals quest.
