I keep visiting the orc in the valley of spirits who tells me without offering me a quest to talk to undead dude out in hillsbrad. Every time I go there expecting to see Triage! available, I see nothing and can only see the first aid abilities I can't achieve beyond the 225 skill cap I'm stuck at.

There is no leader quest for first aid on either faction. As BoxerBriefly says, just go to Hammerfall, and in that little building near the fence gate you should find the good Doctor. You will need to have exactly 225 FA skill and be level 35 for the quest to be visible. (Sounds like you meet those criteria from your post.)

The next chance I get, I'll go ahead and check on the doctor again. I had 220 First aid. I didn't know you needed exactly 225, but that is now solved
