Server fragmentation bug?

I was helping a lowbie priest do the quest "An Unholy Alliance" just outside RFD.
I charged Ambassador Malcin and started killing him and the adds around him when everything despawned and respawned at full hp, according to my partner I also vanished from his screen and healing range.
After killing the mobs I reappeared to him but as he ran in to loot the head he vanished and everything spawned on top of him and killed him.
