Constant lagspikes with Vertical Sync on

When I have Vsync off I get 200+ FPS everywhere constant and everything is really smooth. But when I turn on Vsync my FPS can't hold 120FPS (I've got a 120hz monitor so that's the FPS it should stay on) it keeps dropping down to 60-100 as I'm running around and it feels really choppy.
Does anyone have a solution to this? Also my castbar lags like hell all the time. It's very choppy, not at all smooth like it should be.
