Which of the options from the Help Request page (in-game) do you choose to report a player breaking the ToS? None of them seem to fit the criteria.

You can use any option that you want. We usually suggest Stuck -> Still Stuck (Page a GM).

The ticketing system does not categorize tickets when a GM sees it based on what options you select. The categories are just a relic of Blizzard's support system, which we are not using.
