[BUG] x 2 (Blackfathom Deeps)

1st one is in the twilight cultist area. We were fighting our way toward Serra Kis when a few of the mobs "warped" through the plateau into the water and started fighting us. We killed all of them except for 1 runner, who then swam through the floor of the plateau and pulled the room (yeah we whiped)
The second one relates to the quest given by the drop from Baron Aqaunis. I was able to start the quest a second time (having previously completed it). I turned the quest in a second time for no XP, but I did get the 1h again. (so now I have 2)
Not sure if that quest will work infinitely, I'm running BFD again this weekend so will update on whether it was a one time thing or if it goes on.
