Permanently phased out, disconnect upon any action.

This started when I hearthed to Kargath from an area in Badlands. I immediately get disconnected, then I come back still in the same spot before I hearthed and my hearthstone is on cooldown. I ride to Kargath and then get disconnected again, as I did not see any NPCs or players. I log back in and fly to STV, disconnected several more times after that by using and action or trying to /log.

If you need any more information please let me know, after exiting the client the problem still persists ONLY for this character, it is fine for any other character.
