Nostalrius Begins - Quality wow vanilla realm (1.12)

Fam'retor Guardian will not drop the bindings when killed, and can be spam summoned through repeatedly abandoning + accepting the quest to grief other players. This is confusing because i've seen people with the trinket. What's up with this quest + npc?? ((
