Have EVERY attack trigger guards in neutral towns.

And I mean EVERY attack. Anything that puts either you or your target into combat should trigger guards in Booty Bay, Ratchet, Gadgetzan and that one in Winterspring no one cares about.

Any form of attack on a person in these towns should trigger guards. No exceptions.

Interesting experience in Ratchet. I cannot for the life of me remember whether this is how it worked in retail vanilla or not.

I'm waiting for the R-BB ship to come in. An orc warlock is afk next to me, her VoidWalker left on aggressive. The pet attacks me, guards kill the pet and then her. I laugh. A troll shaman decides to exact revenge for this slight against Horde honour, jumps into water and kills me with chain lightning (helps being lvl 44 when I'm 36) before I have chance to run away/shield. Guards did not attack him - apparently it's possible to get far enough from guards if you're in the water.

Anyway, I ressed and made the ship - only Alliance on board. A level 13 player starts whacking at me with his staff for some odd reason, I put up PW:S and at the same moment all the Horde players that were blue to me up to that point, turn red and promptly gank me.

So crying salty tears, I ressed again and took the next boat. Also, I'm so going shadowform at 40 and killing every Horde who ever takes that damn route.

In any case:
1) guards aggro radius might need checking, otherwise griefing at the pier will be way too common.
2) fighting on the ship itself - is it supposed to happen?

Sounds like guards are evading at water instead of swimming/shooting.
