In what situations should tanks not focus on keeping threat and healers not focus on keeping you alive?

In the situation where you are ignoring tank threat completely and/or target order in order to maximize your dps; and standing in the fire or taking other forms of damage that you shouldn't in order to maximize your dps at the expense of straining the healers unnecessarily.
