It happens with zero addons, too. Pet classes far away from you cause this with pets deployed. Why? I don't know. Inconsequential server bug.

The icon has existed since WarCraft 3, when there was no icon for something in the game, you'd get Samwise's face.

That link sort of explains it. The icon is a placehold, it appears when there is an icon error or when no icon exists.

The guy that face belongs to is also ultimately responsible for the Pandarens.

I thought that was my addon acting up. It only happens when the target is out of range so really not to much to worry about.
