It looks quite like the NetherUI which looks slighty different as the chatlog background isnt transparent but black to read better.
Just look NetherUI up on feenix forums. Works only with 1900 resolution though by default but you can tinker with his addons to make it look like his.

I think it's close, but this is still an early development and has a log of bugs/unfinished features.

Anyone else?

You can count UI's like that for vanilla with 1 hand. It probably is just svui with his changes.
