If that isn't the normal way to handle it, then it should be. Otherwise as others have mentioned I'd roll a paladin and by lvl 40 roll need on everything, because hey... I can just change spec right?
All that said, I don't think what Nevah did here is bad enough to blacklist over, but I would be concerned about it.
According to the op he offered to buy the loot rights before he rolled. How much more clearly can he say "I want this loot"?
That is absolutely true and one of the reasons I don't think he should get blacklisted. Honestly I'd gladly play with Nevah after hearing this story, but I would be slightly more nervous about him ninjaing than a random other person with no record.
On the person mentioning leader role for dungeons: Leadership is for marking creatures, calling breaks and tempo of the team. It shouldn't be used to decide who gets what loot. That should be a decision the entire group has a say in.
To save time it is useful to have a standard assumption about how loot is devided in dungeons, and honestly, the most fair system in my mind (and most people I've played with) is needers before greeders and current spec/role over off-spec/off-role. If an item doesn't clearly belong to a single role/spec, then it should be equally open for all needers.
Of course people are free to make up their own systems, but if it deviates from the standard system (and I do think what I wrote above is pretty standard) then it needs to be mentioned and discussed before the dungeon starts. And naturally this doesn't apply to people who have a clearly defined alternate system that everyone have agreed to beforehand (such as guild raids and all friend groups).
On the topic of "reserved items": I think it is a douchebag move and I'd try to avoid groups who use this system if at all possible. Of course if it was an equal deal where everyone got to reserve an item I might be okay with it. I will say, that if it has been clearly stated up front that the group uses the system, I'd still be annoyed by it, but I could live with it.
