Achieving Reputation

Having experienced retail since vanilla, I've been dedicated to reputation farming for most of my WoW career. The removal of many factions later on made me wish I had a second chance, and Nostalrius is that second chance.
However, Nostalrius also has various (vanilla) traits that change the face of reputation grinding, so I'm very curious to find out how to get maximum reputation with various factions.
Horde/Alliance:
Doing all quests in all zones give a hefty sum of reputation if you're the appropriate level, but outleveling gives a hefty penalty. I believe Exalted shouldn't be possible unless you have an massive amount of Runecloth or Alterac Valley is released, is this correct?
PvP Forces:
Simple grind and is bugfree.
Steamwheedle:
Outleveling means grind spots are far less efficient at level 60. Reputation rewards at appropriate levels are very time-efficient however. Free Knot is the only available opportunity at level 60. Is there a more efficient method I'm missing?
Others:
Is it possible to use the party trick to gain reputation without losing any? If so, what are the most efficient methods of doing it, assuming multiboxing is disallowed for this (which is how the first character to get all factions maxed out did it)?
Without this bug, opposing factions such as Centuar Clans and Syndicate/Ravenholdt are unable to be min-maxed for reputation.
