This UI is just the sort of style/layout I'm looking to create for myself. Unfortunately, I do not use the same resolution. However, if you would be kind enough to provide me a list of the addons you used to create your UI, it would be very much appreciated. Cheers, and thank you in advance.

name addon where you see raid and player/target portrait?

I'm not sure what you really mean, but I use the addons Luna and Raidhelper for the player/target/raid frames.
If you mean the warrior class portrait on the player frame, that's part of Luna.
