How accurate is KTM? What is "Aggro Chain"? I've seen people completelty over the tanks yet they ha sent pulled Aggro. And still no idea what Aggro Chain is.

It's impossible in vanilla to get an accurate Aggro-value.

KTM is using guessed values based on testing during retail, backporting a newer version wouldn't make sense.
and I doubt anyone here wants to spend as much time as needed to enhance those KTM values because it's not just about changing a few values, it's about keeping the balance on all spells, classes and so on. There is an improved version from Mesmerize from Feenix though, never looked at the code but I guess they adjusted some values but it won't make a big difference.
