Banned for unknown reason

Hi,

My character name is Falz on the PVP server and my account appears to be 'banned permanently' for reasons beyond me. I've not received any emails or notifications regarding the ban.

I'd appreciate any response or assistance the staff at Nostalrius can offer.

I had been suspiciously logged off a couple times in under an hour a few days earlier while running UBRS. I went to change my password but couldn't find the location to do so, and the password reset form I clicked from my email wasn't loading properly. I was only disconnected suspiciously one additional time before leaving for vacation (I've been away since March 18th), but my characters gear and inventory were always left in tact post-disconnect, so I wasn't too concerned. It turned out my guild needed me for BWL on Sunday so I tried logging in and found myself banned.
