It's really more of a personal challenge, if you cheat, you're only cheating yourself.

It's not like theres an achievement for it.

Sounds like an insanely tedious way to ruin your wow experience . Has anyone ever done this? You would need to be extremely careful all the time, especially for respawns, and carry potions with you always. I guess hunter would be the best class to do this.

I wish all the people that want to do this the best of luck! But I'm never doing it.
Will I track the lvl at which I suffer my first death? Sure. Will I wear subpar equipment? Hell no!

Guess you'd have to play alone all the time. Most deaths happen in party (dungeon or group/elite quest).
I'd find it interesting but I won't do it with my first char
