Its like some people stated earlier, its whatever is easiest to type and remember. Every server (retail) had different names for different bosses as well. The most clear example that we see VERY often is people referring to "The Dead Mines" as VC or DM. People like to argue that VC is the correct term because DM could stand for Diremaul... But in my opinion.... Who the hell looks for a Dire Maul group in WESTFALL.

Probably the right answer here.
I tend to look for patterns or reasons for a good amount of things, and sometimes I skip the fact improvisation and on-the-fly ideas are so common.

"God save the Queen" not "God save Elizabeth II". I barely ever see royalty regarded without their title and only if you talk to someone from a different country do they use names instead of status. Most other countries don't change leaders once every few years, unlike the US with their electoral system every 4(look how long the Queen has been the monarch). This is why they have a habit of saying the title instead of name when talking in third person.

You do realise the Queen is a puppet and they have a prime minister?
