The same way you do it with regular MaNGOS for vanilla these days. It works with proper configuration, decent hardware and fixing a few bugs which cause crashes but are easy to resolve. Some of these are code related others are related to issues in the database where the core lacked error checking. No magic involved, just being able to debug the core, and it also helps e.g. to be able to record client traffic and replay it on development systems to watch bugs in action. Also, tools are free and available for that too.

What most people do not understand is that most servers run on MaNGOS versions from 2010, same as Nostalrius did. Kronos also does. Easy to see btw as there are characteristic issues to these versions not existent in current MaNGOS. Most of the good stuff was built starting 2013 and the MaNGOS now is so much different and more capable.
There is not much to do, except to use an up to date version from 2016. It has been tested with thousands of players by the team, it works. There are way too many false rumors about MaNGOS and most stem from public servers. Bash what you use to distinguish yourself. Common thing in the scene.
